Our Offering
We identify partners, providers and locations for community events that equitably serve the population with health care services and health education. We manage the coordination or care and data management to improve and track health outcomes.
Current Programs
The Virtual Dental Home (VDH) model is an innovative approach to providing dental care, particularly to underserved populations who may have difficulty accessing traditional dental services. It leverages telehealth technology to connect dental professionals with patients in non-traditional settings, such as schools, community centers, nursing homes, and other community sites.
